Group Travel Program Director Travel ApplicationAll faculty, staff, and graduate student teaching and directing non-credit Group Travel programs are required to complete this travel application with UK Education Abroad. This application allows the UK International Center the ability to provide the necessary health and safety support, including travel medical insurance. Additionally, in compliance with federal regulations regarding the Jeanne Clery Act, program directors must supply specific information about participant housing and rented space so that the UK Crisis Management division may collect the necessary information regarding local crime statistics.*

Who should submit this application?
This travel application must be completed by each individual with a leadership role in the program. Any person (faculty, staff, or graduate student) who will have the authority to make decisions on behalf of the program and program participants MUST have an application on file with UK Education Abroad. Accompanying faculty, staff or adult community members who will not have a leadership role in the program will submit a Group Travel Participant application, not this program director application. Additionally, any person (faculty, staff, or graduate student) who will have the authority to make decisions on behalf of the program participants MUST complete the Campus Security Authority (CSA) training as designed by UK Crisis Management. Any program director who has already completed the CSA training in a given calendar year as part of their regular job responsibilities will not need to complete the training again. If a program has multiple directors, the required academic and Clery Act information in this application need only be submitted once, by the primary program director.
This application is unique and exists only in a given semester, year, and geographic location. Therefore, a new application must be submitted each time a person coordinates a group travel program. Applications do not automatically roll over from year to year. The contents of this application are confidential and access is restricted to certain UK International Center staff.

The following items will be submitted electronically to this application and should be submitted no later than 4 weeks prior to program departure.
